Community Engagement

Our Community Engagement Services are designed to support waiver participants in building
meaningful connections within their community. The focus is on inclusion, independence, and
access to natural supports that help individuals work toward their personal goals.
Rather than providing direct, ongoing support, our role is to connect participants to
community-based resources that align with their interests and goals. This approach
encourages long-term participation, confidence, and belonging within the community.
 

What This Service Looks Like
We help participants:
● Explore interests and identify community activities that align with their goals
● Locate inclusive, community-based programs and resources
● Assist with registration, scheduling, and navigation of services
● Provide initial support to help participants get started
● Gradually step back as participants build informal and natural supports

 

 

 

 

 

 

 

 

 

Our Approach
Community Engagement Services are not direct support services. The purpose is not to
replace community participation with 1:1 support, but to empower participants to engage
independently within their community.


Example Goals & Supports
Goal: Improve physical health and wellness


If a participant is interested in:
● Hiking: We assist in locating hiking groups, support sign-up, and provide short-term
support to encourage independent participation.

● Bowling: We set up dates/time for bowling, create sign-up sheet and reserve bowling lanes.

● Sporting Events: We look for local sporting events, such as baseball, football, and soccer games. Offering the opportunity to witness local sporting events live. 

What Happens Next
Our goal is to help participants feel confident and connected in their community. If additional
support is needed after community connections are established, families and support teams can
work together to identify appropriate next steps.
